Health Questionnaire
This nutritional analysis starts with an in-depth questionnaire, done in the comfort of your own home, giving
the Doc information on specific health concerns you (or your child) may have and how they are affecting your
life and well-being (or your child's if a parent is doing it for them). This information gives her a picture of what
is going on with your child's or your own body. The answers are then charted on a graph to use in the
evaluation. The cost of this evaluation is only $30!

Doc McCullen then goes over your answers to each of the questions and correlates those results with that of
the graph and puts together a whole food nutritional and wellness program based on her extensive education
and prioritized for your child's or your individual needs. This service is available locally in the Tampa Bay

There are actually 4 different tests - ideally you would do all of them as that will give the Doc the most data to
work with in helping you with all your healthcare needs. But if you don't have the time to do all of them - read
the descriptions below and do the one that you feel applies to your immediate situation. You can always come
back later and fill out the rest.

Medical History

We ask you to fill out the Medical History as this is the background of what has happened to your body.
Knowing the past, and what has brought you to this point, it gives a basis for future recommendations. There
was a brilliant doctor who did a study on cats, and in the field of nutrition  it has become know as Pottinger's
Cats. The study gave us tremendous insight into what the effects of your parent's health and your
grandparents health is on your body and the way it should be treated. It makes a huge difference.

Health Appraisal

Who knows your body better than you do? Really no one. You just have to be asked the correct questions to
be able to share what you already know. Symptoms show up considerably before there are changes in blood
work, urinalysis, or any other lab test. Several world class nutritionists realized that we can start your return to
excellent health BEFORE you are pathologically showing symptoms on those tests. And how to do this
uninvasively? Ask the correct questions. Over to you for the answers.

Symptom Survey

Now we are computerized - super dooper technology. Again we have your input, and now we have great
matching of world class supplements to the data that you share. This is a clarifying step, and if my friends and
family want to know about their nutrition I say please do them all. But if you are really really pressed for time,
and want the quick answer - do this one. The answers are great, and frankly, it is the easiest for me!

Toxicity Test

A very basic place to start any body improvement plan is do you need to clean house first. Would you start
painting kitchen cabinets without cleaning and sanding them. You could, but the result would be less stellar
than if properly perpared. And if the answer is a resounding "Yes - Clean house first!" then this is the fastest
way to get started.
